Better Life Grievance Procedure

A fair and accessible grievance resolution process exists for individuals accessing, receiving, or being evaluated for services at Better Life Therapy. Notices of dissatisfaction about any matter related to our services are accepted verbally or in writing.

Please prepare an email for (preferred) or request the phone number of our Clinical Director, Stephanie Berliant, with any concerns. Please include the following information:

  • Your name
  • Date of birth (if currently a client)
  • Phone number
  • Email address
  • Time of submission
  • Current date
  • Location of event
  • Witness names (if applicable)
  • Your account of the event
  • Proposed solution
  • Please sign and date before submitting.

To request our Clinical Director’s phone number or email address, reach out to our scheduling desk: 720-445-9979 / betterlifecolorado@gmail.com. Simply request her contact information and state that you would like to file a grievance. Her contact information will be provided right away, no questions asked.

The Clinical Director will provide a resolution within fifteen (15) business days.

Better Life clinicians are regulated by The Colorado Department of Regulatory Agencies.
